We accept payment for your visit from your insurance carrier with the balance due to be paid within 30 days. We require the deductible, copay, co-insurance payments and non-covered fees be paid at each visit. In the event of duplicate payment you will be reimbursed.

We accept the following insurance plans; however, please call your insurance company to verify your coverage and see if we are in-network. We do not accept Medicare, Medicaid, Trillium or Oregon Health Plan.

Self-Pay Options

  • We do accept HSA/FSA for payments

  • Appointment payments must be paid at time of service with checks, credit cards, or HSA/FSA cards. No cash payments accepted. 

  • If you have billing questions, please ask the office staff.

  • We charge a missed appointment fee if there is less than 48-hour reschedule notice. Please read the FAQ’s for more specifics.
